In order to remain compliant with FICA requirements, Metropolitan Health Group (MHG) is required to obtain the banking details of all our registered service providers.
Kindly provide us with the following documentation by no later than 30 July 2010:
- signed practice letterhead reflecting your practice number
- certified copy of the identification document (ID) of the practice owner or registered doctor
- cancelled cheque

- letter from your registered banking institution confirming your banking details or
a copy of your latest bank statement (not older than three months).

As failure to provide this documentation may result in the suspension of your claims payments, we would appreciate your co-operation in this regard. |
